Supportive Living Services (SLS) is a supervised apartment program for the catastrophically-impaired brain injured who are potentially capable of achieving a supported independent lifestyle. The program provides these individuals with the opportunity to maximize their skills in a structured, emotionally-supportive environment. SLS is designed to address every resident's personal requirements with each Plan of Care individualized to balance personal control and goal direction with supervision, guidance, and assistance.

What Makes Us Outstanding

SLS is small and desires to remain small. With a maximum of 20 residents, the SLS team is able to gain a more personal understanding of each individual, creating the ability to construct a unique, caring, and supportive environment.

Our small size enables management to share in the day-to-day activities of the residents and participate in all case manager/guardian meetings. Prompt resident reports are concise and insightful since the author participates directly in resident activities. In addition, no contracts with auxiliary services are established; this, allowing more input from team members such as case managers, guardians, physicians, and work sites.

QUALIFIED TEAM

The Owner/Administrator, Dean Prappas, (MBA) and the Medical Coordinator, Linda Swagler, are the SLS core team with over 30 years experience. Both Dean and Kathy Prappas, and Linda Swagler live the philosophy of service to their residents. Available 24/7, they are physically present throughout the day at the SLS complex. The remainder of the team forms a cadre of individuals who are committed to our residents' growth through caring service and support.

Companionship is key to a trusted relationship with our caregivers. We not only strive to help you with everyday tasks but want to develop a caring relationship with you. We provide one-on-one attention and care that cannot compare in other settings.
