We provide a wide range of services

Unlike most traditional apartment programs, SLS provides 24-hour on-site staffing for continual monitoring, supervision, and training of its residents. This provides the potential to immediately respond to the needs of our residents. Immediate responses are a key ingredient to reduce anxiety and limit emotional stress.

SLS offers the availability for transportation services to all medical and rehabilitation related appointments. Our program also makes transportation available to all residents for social, leisure, and recreational activities along with prescribed day programming activities.

SLS has cost-effective per diem pricing which decreases the out-of-pocket expense to our residents and stakeholders. Per diem pricing includes extras such as grocery allowances, cable TV, phone, and transportation.

Inclusive Services

Room & Board

  • Private and Semi-private apartments

  • Fully-furnished living room, dining room, and bedroom(s)

  • Washer/dryer

  • Small appliances, linens, dishes, silverware, glasses, bakeware, pots and pans

  • Basic cable, telephone, water, heat, and electric

  • Grocery (up to $60/week)

  • Live in significant others and/or family members will be considered

Medical & Case Coordination

  • Assistance in scheduling medical appointments and maintaining client medication requirements

  • Transportation to medical appointments

  • Follow-ups with physicians and case managers

  • Documentation of physician appointments

  • Program consultation with staff psychologists/behavioral therapists

  • Medication dispensed upon physician’s orders

  • Case management

Community/Leisure

  • Assistance in planning individual/group activities off campus

  • Support for community job developement (if appropriate)

  • Community support groups for brain injury and substance abuse education

  • Social transportation available

Staff
